STHL Serves Two New York Medicaid Health Home
Programs
Southern
Tier HealthLink (STHL) is pleased to support two NY Medicaid Health
Home projects in our community. Catholic Charities of Broome
County and United Health Services will each lead their own community
network of health care and behavioral health providers, as well as
social service organizations to provide comprehensive services to
select Medicaid beneficiaries in Broome County. Both projects are
expected to begin accepting patients in January 2013.
The
Health Home is a care management model
whereby
all of an individual's caregivers communicate with each other in order
to address patient needs in a comprehensive manner. Communication
and coordination is accomplished through a Care Manager, who works with
primary care, specialists, and behavioral health providers, as well as,
social service organizations to assure that patients can access the
care and resources needed to stay healthy, out of the emergency room
and out of the hospital. Ultimately, the Health Home model of
care is expected to reduce Medicaid expenditures for emergency room
visits and hospital stays, while providing better, more comprehensive primary
care.
STHL will
play an important role in the program. STHL will provide health
IT services to the lead agencies and their community partners, enabling
care coordinators to more readily track patients' progress, communicate
with each patient's providers, and follow up with the patient.
Catholic Charities and United Health Services each plan to take
advantage of the wealth of clinical data available through STHL's
Health Information Exchange, as well as specific services STHL offers.
These services include access to STHL's Web Portal, document exchange
between STHL and electronic medical record systems, and care management
services, including alerts/notifications following ER visits and a
secure messaging system.
These
services are also available to the medical community at large.

STHL Announces Partnership with the New York eHealth
Collaborative (NYeC)
On
October 4th, 2012, Southern Tier HealthLink NY (STHL) announced its
new partnership with the New York eHealth Collaborative (NYeC) and
its Statewide Health Information Network of New York (SHIN-NY)
Service Platform, which aims to create the infrastructure to connect
health information across New York State. This will make a truly
meaningful difference for patients in this rural community, who seek
care from larger, specialized medical centers.
STHL,
which serves the Broome, Chenango, Tioga, Delaware and Otsego
counties in upstate New York, brings its extensive network of
providers from its health information exchange (HIE) to the
SHIN-NY, along with a functioning patient portal serving 25,000 area
patients. STHL is currently the top performing Regional Health
Information Organizations (RHIO) in the state, in terms of the
highest number of transactions between providers and hospitals
occurring over its network. STHL has also demonstrated great ability
to implement innovative, patient-centered enhancements to their
platform and will be sharing these strengths with other RHIO's as
part of this partnership.
"Our
reasons for joining the SHIN-NY are simple: We can consolidate costs
and make better use of our grant dollars; we can collaborate more
fully to build on strong ideas and execution; and -- most importantly
-- this partnership allows us to reach more providers,"
explained STHL Executive Director Christina Galanis. "In short,
we can offer more at a lower cost to our community."

STHL Attends the NYeC 2012 Digital Health Conference
New
York eHealth Collaborative (NYeC) held its 2012 Digital Health
Conference (DHC) on Monday and Tuesday, October 15 & 16th in New
York City. Christina Galanis, Executive Director and Kirsten
Griffin, Project Coordinator represented Southern Tier HealthLink
(STHL) at the conference.
The
event was well attended by over 900 healthcare professionals from all
different facets of health IT including technology vendors, hospital
officials, physicians, other Regional Health Information
Organizations (RHIOs), group practice managers, entrepreneurs and
more.
DHC showcased practical, innovative, and inspiring
advancements in health information technology where attendees were
able to network, exchange ideas and gain insights on new and existing
innovations in health IT, as well as analysis from leaders in the
health IT community.

STHL Attends Broome County Disaster Preparedness Fair
Did you know
that September was recognized throughout the Country as
Emergency/Disaster Preparedness Month?
On September 26th, 2012, Southern Tier HealthLink
(STHL) was one of over 40 agencies that participated in the Broome County
Disaster Preparedness Fair held at the Binghamton
University Event Center.
In a
disaster situation STHL is prepared to provide emergency access to
patient medical records in the Health Information Exchange (HIE) for
residents who have been displaced and are in need of medical
attention.
Agencies
such as United Health Services, Lourdes Hospital, Catholic Charities,
Broome County Health Department, and the American Red Cross were on
hand to answer questions and provide handouts and information to
assist in disaster preparation. Interactive presentations were
also held for Seniors, Students and Families, as well as pet owners,
providing information on what to do and where to go for help when
disaster strikes. With approximately 300 people in attendance, it was a
great opportunity for residents to learn and understand how to
better prepare and recover in the event of a disaster.

Meaningful Use Resources
and Health Information Technology Timeline
Southern Tier
HealthLink (STHL) strongly supports the adoption and use of
electronic medical record (EMR) systems which combined with health
information exchange (HIE) technology can facilitate the
exchange of health information across organizations, reduce costs,
and ultimately reach improved outcomes in the quality and safety of
patient care. Health Information Exchange (HIE) is a catalyst
that can 'connect the dots' and also encourage greater patient
engagement whereby patients are more invested in their own wellness
and health care.
With
Meaningful Use Stage 1 in full swing, and Stage 2 set to begin in
2014, the Centers for Medicare and Medicaid Services (CMS) offers a
large menu of helpful resources to physicians and hospitals alike to
help toward meeting Meaningful Use. Make sure to visit the EHR Incentive Programs
web page for the latest news and for eligible
professionals (EP) and eligible hospitals (EH).

to help you navigate the Incentive Programs. By subscribing to
the listserv you will receive information that can help keep you
aware of deadlines and programs that affect you and your health care

CMS has published a comprehensive timeline surrounding
Health Information Technology (HIT) programs and initiatives through
2013, including Meaningful Use, as well as an Attestation Calculator
to determine whether or not you are ready to demonstrate Meaningful
Use.
